Call us at (276) 403-9070. Tell us a little about why you're looking for a new doctor — established care, switching practices, or something specific.
We'll get you set up.
Our front-office team will collect your insurance info, ask a few short questions, and schedule your first visit. We'll send over a packet of intake forms ahead of time so you can complete them at home — not in the waiting room.
Come in, meet your doctor.
Your first visit is a longer one — usually 45 to 60 minutes. We'll go through your history, your goals, your current medications, and any concerns. You'll leave with a plan, not just a prescription.

What to expect.
No surprises. Here's how the first appointment usually goes.
- Check-in (about 15 minutes). ID, insurance card, and the intake forms we sent you. If anything's unfinished, the front desk will help.
- Vitals. Blood pressure, weight, height, heart rate. Maybe an EKG or basic lab work depending on your history.
- Time with your doctor (45-60 minutes). A real conversation, not just a checklist. We'll go through your history, current concerns, and goals.
- A care plan. Together we'll decide on next steps — labs, imaging, prescriptions, lifestyle changes, or specialist referrals as needed.
- A follow-up. We'll schedule your next visit before you leave (annual physical, recheck, or whatever's appropriate).
What to bring
Make the first visit count.
A little prep helps us understand your full picture from day one.
- Photo ID and insurance card. Bring both, even if we already have them on file from urgent care.
- Completed intake forms. We'll send these ahead of time. Filling them out at home is much easier than in the waiting room.
- List of current medications. Include over-the-counter meds, vitamins, and supplements. A photo of your pill bottles works.
- List of allergies. Especially medication and food allergies — be specific about reactions.
- Records from your previous doctor. If you can request them ahead of time, that's ideal. Otherwise sign a release at check-in and we'll request them.
- Your questions. Write them down so you don't forget. We've got time.
Already a patient upstairs?
Switching from urgent care to primary care.
If you've used Martinsville Urgent Care before, we already have your basic info on file. Establishing primary care is straightforward — and your records stay in the same building.
- We can pull your existing record from urgent care so you don't have to re-share basic info.
- If you've been seen for an acute issue (sinus infection, sprain, physical), we already know who you are.
- Primary care visits are scheduled, not walk-in. You'll see the same provider each time.
- Just call and tell the front desk you'd like to establish primary care. We'll take it from there.
