What to bring.

Walk-ins are always welcome — but a few items will help us get you in and out faster. Here's what to grab before you head out the door.

Essentials

Always bring these.

If you're between insurance, between meds, or just unsure — come anyway. We'll work with what you have.

  • Photo ID. Driver's license, state ID, school ID, or passport — anything with your name and date of birth.
  • Insurance card. If you have one. We accept most major plans (Anthem, Aetna, Cigna, United). No card? Ask about self-pay rates at check-in.
  • List of current medications. Names, doses, and how often you take them. A photo of your pill bottles works.
  • List of allergies. Especially medication allergies — be specific about reactions when you can.
  • Method of payment. For copays, deductibles, or self-pay. We accept credit, debit, HSA/FSA, cash, and check.

Helpful extras

If you have them, bring them.

These help us understand your full picture, but none of them are required to be seen.

  • Immunization records. Especially helpful for kids, sports physicals, and school.
  • Records from another doctor. Recent test results, imaging, or a note from a specialist.
  • The injured item. If a foreign body is involved (a splinter source, a hook, a piece of glass), bring it if you can — helps us know what we're looking for.
  • For workplace injuries: the name of your employer and any incident report or claim info from your manager or HR.
  • For DOT physicals: glasses or contacts (if you wear them), hearing aids, and a list of medications.

Virtual visits

Have a telehealth visit coming up?

A few minutes of prep keeps things smooth.

Not all symptoms qualify for telehealth visits.

  • A device with a working camera and microphone (phone, tablet, or computer)
  • A reliable internet connection — Wi-Fi works better than cellular if you have a choice
  • Good lighting (sit facing a window or a lamp, not with a window behind you)
  • A quiet, private space
  • The same essentials as an in-person visit: medication list, allergies, and the symptoms you're seeing
